Missing details on Treatment Plan
Without a plan it appears the doctor is ‘winging’ it. The patient needs to know what he is expected to do.

Poor Progress Notes
Without good notes it is impossible to justify your fees. No longer can you mark “C5-C6”. Repetitive notes looks real bad on an 8 by 10 foot screen in the court room.

Little of No Evidence of Home Rehab
This is a killer. Judges and juries respect home care programs. If the DC does not have one—diminishes the case.

Lack of Evidence of Exercise Rehab
All trauma patients need an exercise program in the office. Using specialized rehab machines provides excellent proof.
