Dr. Gary Lewkovich recently examined the faulty grounds many IME [insurance doctors] use for criticizing care.
Examples of attacking the practitioners work include:

✓ | Denial of payment after 18 visits based on the Official Disability Guidelines. |

✓ | Denial for a diagnosed cervical radiculopathy based on the same guidelines. |
These and other examples are not credible, ethical or reasonable. Sadly, too many insurance doctors are intentionally misreading the literature to attack honest diagnosis and treatment protocols.

II. | Slight to moderate - desiccation with disc space narrowing or slight disc bulging. |

III. | Moderate - moderate size disc bulging or Small central disc herniation effacing the Thecal sac. |

IV. | Moderate to severe - poSteromedial disc herniation effacing the thesal sac. |

V. | V. Severe - Extended disc herniation. |
